Summary In collaboration with the entire Community Employment Team; ensure quality and exceptional training services are provided for individuals with diverse abilities while expanding their employment options in the community. This is accomplished through direct, hands-on training protocols, in addition to case management services funding coordination. Schedule will include evenings, weekends with varied hours. Services to be implemented following agency procedures, and all state and federal funding requirements.
Essential Duties and Responsibilities include the following:
Training Services/Coordination: (approximately 75% of the position’s activities) • Provide job coaching services for participants at community-based job sites, based on training team needs and individual plan requirements, following the training protocols outlined in the Engagement/Training/Coaching Guide. • Utilize various tools including the customized on-line career exploration and training system through Opportunities, Inc. to advance knowledge and skills of youth and adult participants. This will be done in both individual and group settings (i.e. classroom). • Training Practices: o Assist participants with adjusting to the work routine and procedures; provide specific training or as needed for such things as relating to supervisors, absenteeism, shop rules, peer interactions, etc. o Model appropriate work behaviors, rules, guidelines of integrated employment settings. o Provide direct work skill training and support according to individual training plan, to include: ? orientation to new work jobs ? implementation of one on one individual training ? implementation of group training initiatives ? direct hand on hand training that demonstrates the work skill outcome (will require continual repetition and monitoring) ? implementation of specific protocols or motivation plans that: ? reinforces skills based on the needs and abilities of the participant ? addresses barriers and other identified challenges ? assistance with personal needs and cares as supported by the participant’s plan
• Communication and Outcome feedback o Maintain positive and efficient communication following the KEYS to SERVICE SUCCESS o Provide suggestions for improvement opportunities – attendance, job performance, safety. o Provide regular feedback to the participant about skill progress/advancements o Provide regular feedback to the participant about skill concerns/issues o Provide feedback to the appropriate training team member regarding participant progress, needs and/or concerns/issues. o Maintain notes, written observations and other designated forms of documentation for participants o Assist in the development of the participant’s Individual Service Plan, Protocols and Motivation Plans o Document training, improvement opportunities, and attendance issues in the employee record.
Communication/Activity/Reporting/Billing- (approximately 20% of position’s activities) • Follow the agency service delivery system in all communication called KEYS to SERVICE SUCCESS. • Maintain full on on-going communication with participant, chosen support network, employer (if appropriate) and funding/referral source until service is completed. Focus on face to face and phone call communication with email and other written options as secondary documentation whenever possible. • Arrange and coordinate meetings using GAIN protocol, with participant and support network when needs arise, (i.e. DVR counselor, parents, guardians, employer, social security representatives, in-house staff, etc.) and to transition successfully to long term support. • Manage funding and referral purchase orders to ensure contracts are completed as expected. • Write regular formalized reports which will include placement plan, intake information, monthly progress notes, follow-up reports and discharge summaries that meet the requirements of agency and funding. • Maintain updated and thorough activity records and billing in Apricot (system) to properly document services provided on a monthly basis.
Team Support/Expectations – (approximately 5% of position activities) • Partner with Employment Specialists on various business development and employment connection tasks to ensure participant outcomes are met, i.e. job shadows, work experiences, job placements, on an as needed basis. • Participate in training opportunities as needed to advance the implementation of service delivery, related to the following (DVR, REDI, School contract services, Community Employment Services, Agency Initiatives, etc.) • Actively participate in all Team and Agency Meetings. • Required to maintain a valid driver’s license and acceptable driving record (based on agency policy) to allow for the participant transportation, along with training and development assignments. • Other duties as needed to expand mission.
Complete expected Key Performance Indicators monthly: • Maintain a caseload of 35 – 40 active participants (may fluctuate depending on service). • Document 128 hours of individual service activity per month (billable and non-billable) • Bill $5300.00 per month in personal direct service delivery. (extended amount for SKT) • Strive to attain Training Team Incentive Plan opportunities through Business Connection Objectives. (See TS Key Performance Indicators)
Service Requirements Due to community based position, personnel are required to hold a valid driver’s license and maintain a driving record in good standing with company policy, along with a mobile device of communication. • Must be willing to travel to and from community service sites and work flexible hours. • A mobile device is required to maintain communication while in the community. This must be available for work hours in accordance with the agency cell phone/mobile device policy.

Education and/or Experience Bachelor's degree (B. A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
Language Skills Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Mathematical Skills Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Reasoning Ability Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Computer Skills To perform this job successfully, an individual should have knowledge of Microsoft Office 2007 Professional Word, Excel, and Outlook software. Should have knowledge of Microsoft Windows 2000 Server.
Certificates, Licenses, Registrations TB test required annually.
Other Qualifications Good driving record, travel up to 75% of the time with flexible work schedule.
